Job Summary

CVS Health is seeking a highly skilled Nurse Practitioner or Physician Assistant to join our Primary Care team. As a key member of our clinical care team, you will provide evidenced-based care to a panel of patients, working collaboratively with a dedicated team to drive better outcomes.

Key Responsibilities
  • Provide accurate assessment, diagnosis, treatment, and management of health problems, as well as health counseling and disposition planning.
  • Counsel and educate patients and families on health maintenance and respond to patient care inquiries.
  • Evaluate and treat patients' medical needs with safe and high-quality treatment.
  • Document all patient care within an electronic health record in accordance with current policies and procedures.
  • Work as a member of the clinical care team, providing care and coordination of patients with internal and external colleagues to ensure the highest level of care.
  • Provide patient-centered care that is compassionate, appropriate, and effective for the promotion of health, prevention of illness, and treatment of disease.
  • Refer to appropriate specialists and patient care components as necessary.
  • Collaborate with the clinical care team to ensure the delivery of high-quality, patient-centric, longitudinal care.
  • Participate in and lead clinical care team huddles as needed.
  • Demonstrate core values of CVS Health in all communications and interactions.
  • Communicate effectively with members of the clinical care team, other health professionals, and health-related agencies.
Requirements
  • A minimum of 1 year of recent Primary Care experience as a Nurse Practitioner or Physician Assistant in either a family medicine or primary care practice.
  • Successful completion of CVS Minute Clinic primary care training program, if not having recent experience.
  • Working knowledge and understanding of quality measurement in the management of chronic disease conditions.
  • Ability to work remotely in a clinical care team culture.
  • Ability to work without direct supervision and practice autonomously.
  • Proficiency with information management and technology.
  • Capacity to collaborate with professional colleagues frequently to ensure quality patient care.
