Primary Care Nurse Practitioner/Physician Assistant Opportunity

Job Summary

We are seeking a highly skilled and compassionate Nurse Practitioner or Physician Assistant to join our Primary Care team. In this role, you will work collaboratively with our clinical care team to provide evidence-based care to a panel of patients. Your expertise will be essential in assessing, diagnosing, treating, and managing health problems, as well as providing health counseling and disposition planning.

Responsibilities
  • Provide patient-centered care that is compassionate, appropriate, and effective for the promotion of health, prevention of illness, and treatment of disease.
  • Collaborate with the clinical care team to ensure the delivery of high-quality, patient-centric, longitudinal care.
  • Participate in and lead clinical care team huddles to drive better outcomes for patients.
  • Communicate effectively with members of the clinical care team, other health professionals, and health-related agencies.
  • Work autonomously and make decisions that align with the company's values and policies.
Requirements
  • Master's degree in Family Nurse Practitioner program or Master of Science in Physician Assistant Studies.
  • Current National Board Certification and State of Employment license to practice in the Advanced Practice role.
  • A minimum of 1 year of recent Primary Care experience as a Nurse Practitioner or Physician Assistant.
  • Working knowledge and understanding of quality measurement in the management of chronic disease conditions.
