Job Title: Speech Language Pathologist

Company: Compassus

The Home Therapy Program is a collaborative effort between Compassus and healthcare professionals to provide speech therapy services to patients in their homes. As a Speech Language Pathologist, you will play a crucial role in developing and implementing individualized plans of care for patients with speech and language disorders.

Responsibilities:

  • Participate in the development of speech therapy plans of care for patients with speech and language disorders.
  • Conduct assessments and evaluations of patients to determine their direct care needs.
  • Communicate with physicians and other healthcare professionals to ensure seamless care coordination.
  • Provide education and instruction to patients, families, and healthcare team members.
  • Participate in interdisciplinary team meetings to coordinate patient care and exchange information.
  • Maintain accurate and timely documentation for billing and patient records.
  • Perform other duties as assigned by management.

Requirements:

  • Bachelor's degree in Speech Language Pathology required.
  • Master's degree in Speech Language Pathology preferred.
  • Minimum of one year of experience in a home health setting required.
  • Active and unencumbered Speech Language Pathologist license in state(s) of employment required.
