About the Role:

We are seeking a skilled Clinical Outreach Specialist to join our team at Open Sky Community Services. As a Clinical Outreach Specialist, you will play a vital role in delivering evidence-based interventions and providing support to individuals with mental illness or dual diagnosis.

Key Responsibilities:

  • Deliver interventions identified on the Treatment Plan with supervision from licensed clinical staff.
  • Implement activities that create opportunities for the development of valued roles and personal relationships in the community.
  • Provide side-by-side support and coaching to help individuals socialize.
  • Conduct both 1:1 and group activities geared toward increasing Persons' readiness to engage in rehabilitation.
  • Ensure individuals have adequate financial support through assisting with finding gainful employment or applying for entitlements.

Requirements:

  • High School Diploma, GED or equivalent, required.
  • Valid Driver's License, reliable and insured vehicle, and acceptable driving record, required.
  • Reliability, willingness to learn, and being open to new opportunities.
