Nurse Coordinator

We are seeking a skilled Registered Nurse to join our team as a Clinical Coordinator in Acute Care. In this role, you will have the opportunity to work with a dynamic team of healthcare professionals to provide high-quality patient care.

Responsibilities:

• Provide direct patient care and coordinate with the healthcare team to ensure seamless care delivery.
• Develop and implement individualized care plans to meet the unique needs of each patient.
• Collaborate with physicians and other healthcare professionals to ensure accurate and timely communication.
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.

Requirements:

• Current Registered Nurse (RN) license in the state of practice.
• At least 18 months of registered nursing experience.
• Dialysis experience preferred.
• Basic computer skills and proficiency in MS Word and Outlook required.
