Job Description

As a Nursing Care Specialist - Resident Wellness, your primary responsibility will be to ensure that our residents receive the highest level of care and support. This includes identifying ongoing needs and services, conducting regular wellness visits, and collaborating with the Resident Care Director to assess and document resident health status.

You will also be responsible for maintaining accurate and up-to-date medical records, communicating effectively with residents' family members and healthcare providers, and providing clinical support to community team members as needed.

Responsibilities
  • Identify and prioritize ongoing needs and services of residents to promote high-quality resident services.
  • Conduct regular wellness visits and assessments to monitor resident health status.
  • Maintain accurate and up-to-date medical records, including weights, vital signs, and medication profiles.
  • Communicate effectively with residents' family members and healthcare providers to ensure seamless care coordination.
  • Provide clinical support to community team members as needed, including training and education on infection control practices.
Requirements
  • Graduate of an approved college or school of nursing.
  • Current state license as a professional Registered Nurse (RN).
  • Minimum one year experience in home health, assisted living, hospital, or long-term care environment.
  • Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, and federal, state, and local regulations.
  • Able to work weekends, evenings, and flexible hours, available to our customers at peak service delivery days and times.
