Salary

The salary for this position is competitive, ranging from $65,000 to over $80,000 per year, depending on experience and qualifications. In addition to a competitive salary, we offer a range of benefits, including:

Job Description

We are seeking an experienced and licensed Physical Therapist to join our team at JAG Physical Therapy. The successful candidate will be responsible for assessing patients' strengths and weaknesses and developing effective treatment plans to aid healing and improve functional independence.

Key Responsibilities:

  1. Collects and records patient care data that follows format and guidelines of the patient care service, regulatory agencies, and third-party payers.
  2. Conducts initial evaluations, daily notes, reassessments/progress reports, discharge plans, and communication with members of the team.
  3. Prepares for evaluation in accordance with department policies/procedures and standards of practice.
  4. Selects and administers tests and measures.
  5. Synthesizes evaluation results to formulate diagnosis, goals, prognosis, and plan of care.
  6. Establishes treatment plan based on evaluation results, goals, and best practice.
  7. Collaborates with interdisciplinary team, patients, and patient's support system.
  8. Assesses and re-assesses need for modifications to plan of care and goals.
  9. Identifies need for consultations and/or refers the patient to another healthcare professional or healthcare services.
  10. Initiates and completes discharge plan.
  11. Provides education/training to patient and/or patient support system.

Requirements and Qualifications

  • Bachelor's or Master's degree required; Doctorate degree preferred.
  • Licensed Physical Therapist in the state practicing in.
  • Completed state-regulated CEU requirement in appropriate state (NY, NJ).
  • Valid CPR license.
  • Experience/knowledge with EMR software preferred.
  • Excellent oral, written communication.
  • Strong clinical decision-making skills.
  • Excellent work ethic and dedication to patient success.
  • Basic computer skills.
  • Works as a team member and individually with minimal supervision.
  • Maintains a positive, professional demeanor at all times.
