We are seeking a Physical Therapist to join our team who shares our passion for delivering exceptional patient care and promoting a supportive environment that fosters professional growth and development.
Job Summary
The successful candidate will be responsible for assessing patients' strengths and weaknesses to create customized treatment plans, maintaining patient confidentiality, and working collaboratively with the interdisciplinary team to promote optimal patient outcomes.
Responsibilities
- Collect and record patient care data, including initial evaluations, daily notes, reassessments, progress reports, discharge plans, and communication with team members.
- Develop and implement treatment plans based on evaluation results, goals, and best practices, collaborating with the interdisciplinary team, patients, and support systems as needed.
- Provide education and training to patients and support systems on ADLs/IADLs, therapeutic activities, rehabilitation equipment prescription and training, and community reintegration.
- Oversee support staff, including PTA, ATC, and PT aides, ensuring effective collaboration and communication.
- Promote JAG in the community through lectures and presentations, enhancing the company's reputation and presence in the healthcare industry.
Requirements
- Bachelor's or Master's Degree required; Doctorate degree preferred.
- Licensed Physical Therapist in the state practicing in.
- Completed state-regulated CEU requirements in appropriate states (NY, NJ).
- Valid CPR License.
- Experience/knowledge with EMR software preferred.
- Excellent oral, written communication, clinical decision-making, work ethic, and dedication to patient success.
Salary and Benefits
The estimated salary for this position is $85,000 - $110,000 per year, depending on experience and location.
