Constructive Partnerships Unlimited is seeking a skilled Habilitation Aide to join our team in providing active treatment services to individuals in day treatment and day habilitation settings.
Key Responsibilities- Assist Day Treatment staff in the development and implementation of individual treatment plans for participants.
- Participate in the evaluation of participants and contribute to the development of their individual treatment plans.
- Implement participant treatment plans by following prescribed methodologies.
- Maintain accurate records and charts for each participant, including goal/data sheets, behavioral data sheets, and inclusion forms.
- Assist participants with activities of daily living, including toileting, showering, grooming, oral hygiene, meal programs, and dressing as required.
- Monitor participants' physical well-being and report any changes in condition or health care concerns to nursing staff.
- Accompany participants to and from buses/vans and ensure their safety when getting on and off vehicles.
- Monitor participants' lunches/snacks and report any dietary concerns to nursing staff.
- Accompany participants to emergency room, doctor appointments as assigned.
- Participate in special programs/projects, inclusionary, and volunteer opportunities.
- Attend mandatory in-services and specialized training programs as assigned.
- Prepare and set up classroom for day's activities prior to participant's arrival.
- Maintain cleanliness and safety of assigned work area.
- The ability to coordinate the movement of the arms, legs, and torso together in activities where the whole body is in motion.
- The ability to safely lift individuals to/from various surfaces.
- The ability to safely perform a two-person transfer and stand pivot transfer with weight-bearing individuals.
- The ability to lift and reach common objects such as laundry and nonperishable food items.
- The ability to push/pull, turn, and maneuver equipment including occupied wheelchairs.
- The ability to bend, stretch, rotate, or reach out with the body, arms, and/or legs to ensure the safety of individuals in all situations.
- Completion of OMRDD Approved Medication Administration Course.
- Completion of Cerebral Palsy orientation training program.
- Perform all job responsibilities in full compliance with all applicable laws, rules, regulations, policies, and procedures.
