Responsibilities
  • Transport residents and participants to various destinations, including medical appointments, social events, and community activities.
  • Communicate effectively with residents, participants, and healthcare professionals to ensure seamless care coordination.
  • Maintain accurate records of transportation services provided, including mileage and hours worked.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Participate in ongoing training and education to enhance knowledge and skills in transportation and care coordination.
Requirements
  • High school diploma or equivalent required.
  • At least one year of experience working with the frail or elderly.
