Occupational Therapist

We are seeking a skilled Occupational Therapist to join our team, providing high-quality care to patients in their home environment. As an Occupational Therapist, you will work closely with patients, families, and healthcare teams to develop and implement personalized plans of care.

• Conduct comprehensive assessments to identify patient needs and develop effective plans of care.
• Provide occupational therapy services, including exercises, activities, and education, to help patients achieve their goals.
• Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
• Travel within the designated geographic area, utilizing reliable transportation and maintaining a valid driver's license.

• Current licensure in the State of Minnesota as an Occupational Therapist (OT).
• Two (2) years of experience as an occupational therapist.
• CPR Certification.

This position will work in the field, visiting patients in their home setting. The office is located in Roseville, MN, and the general territory for this position includes the Southwest Twin Cities Metro area.
