About the Role

BAYADA Home Health Care is seeking a skilled Physical Therapy Assistant to join our team in Aston, Pennsylvania.

This is a full-time opportunity to provide high-quality care to adult and geriatric clients in their homes. As a Physical Therapy Assistant, you will work closely with our multidisciplinary team to develop and implement individualized care plans that promote independence and improve quality of life.

Responsibilities
  • Conduct home visits to assess clients' physical needs and develop personalized care plans.
  • Implement manual therapeutic exercises, gait training, and other interventions to improve clients' mobility and function.
  • Collaborate with nursing staff to revise care plans and ensure seamless care coordination.
  • Educate clients and their families on rehabilitative care and activities to promote independent living.
  • Accurately document client care management and services provided using our state-of-the-art touch pad tablet.
Requirements
  • A current Pennsylvania Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
