About the Role

BAYADA Home Health Care is seeking a skilled Physical Therapy Assistant to join our team in providing high-quality care to our clients in the comfort of their own homes.

Key Responsibilities
  • Conduct thorough assessments of clients' physical abilities and develop personalized care plans to address their needs.
  • Implement a range of interventions, including manual therapeutic exercises, gait training, and balance assessments, to promote clients' independence and mobility.
  • Collaborate with nursing staff and other healthcare professionals to ensure seamless care coordination and effective communication.
  • Educate clients and their families on rehabilitative care and activities necessary to promote their health, safety, and independent living.
  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided.
Requirements
  • A current Pennsylvania Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
