Job Title: Case Manager - Refugee Services

We are seeking a highly motivated and compassionate Case Manager to join our Refugee Services team. As a Case Manager, you will work closely with refugee families and unaccompanied minors to provide case management and supportive services to help them achieve stability and self-sufficiency in their new lives.

Key Responsibilities:
  • Conduct home visits and phone contacts with refugee families and unaccompanied minors to assess their needs and provide case management services.
  • Develop and implement individualized case plans to address the unique needs of each family or minor.
  • Connect families and minors to community resources and services, such as education, healthcare, and employment opportunities.
  • Provide crisis intervention services and support to families and minors in crisis situations.
  • Document all case management activities and services provided in accordance with agency policies and procedures.
Requirements:
  • Bachelor's degree in Social Work or a related field.
  • At least 1 year of experience working with refugee or immigrant populations.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Fluency in Spanish required.
