LVN Coordinator, Home Health Specialist

Job Summary:
Sutter Health is seeking a skilled LVN Coordinator to join our Home Health team. As a key member of our team, you will be responsible for coordinating the overall workflow, including staffing and work assignments of Licensed Vocational Nurses (LVNs), assistants, and interns. This role requires strong organizational and communication skills, as well as the ability to work independently and as part of a team.
Key Responsibilities:
- Coordinate the workflow of LVNs, assistants, and interns, ensuring efficient and effective care delivery.
- Provide nursing care under the direct supervision of a registered nurse (RN) or physician, adhering to established standards of care, policies, and procedures.
- Integrate education, technical competence, and patient interaction skills to gain confidence and cooperation from patients, their families, and other healthcare providers.
- Exercise sound judgment and developed clinical skills to implement patient plans of care based on diagnoses in a timely manner.
- Support department functionality by distributing supplies and performing equipment maintenance, testing, and setup as needed.
- Adhere to all local, state, and federal regulations, codes, policies, and procedures to ensure patient privacy and safety.
Requirements:
- Graduate of an accredited school of Vocational Nursing.
- LVN licensure and Basic Life Support (BLS) certification.
- 4 years of recent relevant experience.
Preferred Qualifications:
- Demonstrated knowledge of nursing skills, medical procedures, and equipment.
- Ability to interpret data and instructions in various formats.
- Basic understanding of human anatomy, physiology, pathology, and medical terminology.
- Excellent written and verbal communication skills.
- Well-developed time management and organizational skills.
- General knowledge of computer applications, such as Microsoft Office Suite.
Work Environment:
This position may regularly work with hazardous drugs and requires adherence to USP 800 regulations.

Pay Range:
$40.06 to $52.06 per hour.
