Job Summary:
We are seeking a skilled Nurse Practitioner to join our team at Bluestone Physician Services. As a Nurse Practitioner, you will provide high-quality, patient-centered care to individuals residing in Assisted Living and Memory Care communities. Our unique model of care focuses on preventative, proactive, and tailored care to meet the unique needs of each patient.
Responsibilities:
- Provide on-site primary care to patients within Assisted Living and Memory Care communities
- Manage a panel of 200 patients, with the ability to design and manage your schedule
- Develop and implement care plans appropriate to patients and their desires
- Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs, and Social Workers
- Collaborate with community partners within home care, hospice, and other resources as appropriate
Qualifications:
- Graduate of an accredited Nurse Practitioner Education Program or Physician Assistant Education Program
- Board Certified/Board Prepared through AANP, ANCC, or NCCPA
- Licensed or ability to obtain licensure through the Minnesota Board of Nursing or Board of Medical Practice
- Ability to obtain and/or maintain DEA certification for the State of Minnesota
- Proven ability to communicate effectively with strong verbal skills
- Excellent interpersonal and customer service skills
Preferred Qualifications:
- 2+ years of clinical experience as a Nurse Practitioner or Physician Assistant
