We are seeking a skilled Nurse Practitioner to join our team at Bluestone Physician Services. As a key member of our multidisciplinary care team, you will provide on-site primary care to patients residing in Assisted Living and Memory Care communities.
Key Responsibilities:- Provide team-oriented primary care to patients within Assisted Living and Memory Care communities
- Manage a panel of 200 patients, with an average of 12-16 patients per day
- Design and manage your schedule to meet the needs of your patients
- Develop and implement care plans tailored to the unique needs of each patient
- Collaborate with community partners, including home care, hospice, and other resources as appropriate
- Graduate of an accredited Nurse Practitioner Education Program or Physician Assistant Education Program
- Board Certified/Board Prepared through AANP, ANCC, or NCCPA
- Licensed or ability to obtain licensure through the Minnesota Board of Nursing or Board of Medical Practice
- Ability to obtain and/or maintain DEA certification for the State of Minnesota
- Proven ability to communicate effectively with strong verbal skills
- Excellent interpersonal and customer service skills
