Speech Language Pathologist Home Health Per Diem

As a Home Health Speech Language Pathologist with CenterWell, you will play a vital role in helping patients recover from illness, injury, or surgery in the comfort of their own homes. Our team of dedicated professionals is committed to providing personalized, comprehensive care that addresses the physical, emotional, and social needs of our patients.
Key Responsibilities- Evaluate and direct speech/language pathology services to patients in their homes or facilities
- Develop and review treatment plans to ensure optimal patient outcomes
- Administer and interpret diagnostic tests and therapeutic treatments to address speech, language, hearing, and swallowing disorders
- Provide instruction and training to patients on alternative communication systems and counsel patients, families, and healthcare staff on speech and language development
- Maintain accurate documentation and submit reports as required by the company and facility
- Certification of Clinical Competence in Speech-Language Pathology or Audiology granted by ASHA
- Minimum of six months experience as a speech therapist/speech-language pathologist
- Home Health experience a plus
- Current and unrestricted license
- Current CPR certification
- Good organizational and communication skills
- Valid driver's license, auto insurance, and reliable transportation
