Speech Language Pathologist Home Health Per Diem

At CenterWell Home Health, we're dedicated to delivering personalized care to patients in the comfort of their own homes. As a Home Health Speech Language Pathologist, you'll play a vital role in helping patients recover from illness, injury, or surgery, and regain their independence.
Key Responsibilities:- Evaluate and develop individualized plans of care to address patients' speech, language, and swallowing needs.
- Conduct thorough assessments and provide evidence-based interventions to improve patients' communication and swallowing abilities.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and patient-centered outcomes.
- Provide education and training to patients and their families on effective communication strategies and swallowing techniques.
- Document patient progress and outcomes in a timely and accurate manner.
- Master's degree in Speech-Language Pathology or related field.
- Current certification in Speech-Language Pathology (CCC-SLP) or Audiology (CCC-A) from ASHA.
- Minimum of six months experience as a speech therapist or speech-language pathologist.
- Home health experience preferred.
- Current and unrestricted license to practice in the state of employment.
- Current CPR certification.
- Good organizational and communication skills.
- Valid driver's license, auto insurance, and reliable transportation.
