Job Summary:

The MDS Coordinator reports to the facility Director of Nursing, administrator, and to the Vice Presidents of Clinical Operations and Clinical Reimbursement.

Responsibilities:

  • Responsible for completion of the Resident Assessment Instrument (RAI) in accordance with federal and state regulations and company policy and procedures.
  • Acts as in-house case manager by considering all aspects of the resident's care and coordinating services with physicians, families, third-party payers, and facility staff.
  • Accurate and thorough completion of the Minimum Data Set (MDS), Care Area Assessments, and Care Plans, in accordance with current federal and state regulations and guidelines that govern the process.
  • Acts as an in-house Case Manager.
  • Proactively communicates with Administrator to identify regulatory risk, clinical trends that impact resident care, and any additional information that has an effect on the clinical and operational outcomes of the center.
  • Collaborates with the facility's staff to select the correct assessment of our residents.
  • Demonstrates an understanding of MDS requirements related to varied payers, including Medicare, Managed Care, and Medicaid.
  • Ensures timely submission of all Minimum Data Sets (MDS) to the state database and ensures that the necessary follow-up action is taken.
  • Facilitates the Care Management Process by engaging the resident and all other appropriate staff members.
  • Analyzes QI/QM data with the Director of Nursing to identify trends on a monthly basis.
  • Serves on, participates in, and attends various committees of the facility.

Requirements:

  • Clinical Nursing Home: 2 years
  • MDS Coordinator: 1 year
  • RAC-CT Certification preferred
