Job Title: Registered Nurse Case Manager

At Athelas Institute, we are seeking a highly skilled Registered Nurse Case Manager to join our team. As a key member of our healthcare services team, you will be responsible for providing exceptional care to our clients and ensuring their health and well-being.

Job Summary:

The Registered Nurse Case Manager will be responsible for delegating nursing tasks to qualified staff, interacting with physicians and pharmacy staff, reviewing medication administration and health care records, and supervising and evaluating the competency of medication administration. Additionally, the successful candidate will assist with health-related in-services and orientation of staff, conduct routine health assessments of clients, and provide comprehensive case management services.

Responsibilities:
  • Delegate medication administration practices and other nursing tasks to qualified staff.
  • Interact with physicians and pharmacy staff to ensure accurate and current prescriptions for clients.
  • Review medication administration and health care records to ensure compliance with regulations.
  • Supervise and evaluate the competency of medication administration staff.
  • Assist with health-related in-services and orientation of staff.
  • Conduct routine health assessments of clients.
  • Provide comprehensive case management services to clients.
  • Maintain accurate and up-to-date records of client care.
  • Collaborate with other healthcare professionals to ensure seamless care.
Requirements:
  • Current Maryland Registered Nursing licensure.
  • Preferred experience working with adults with intellectual disabilities in a community setting.
  • Ability to maintain current licensure and certification requirements.
