Job Summary

We are seeking a skilled and compassionate LPN to join our team as a Private Duty Nurse. As a Private Duty Nurse, you will provide high-quality care to our patients in the comfort of their own homes.

Key Responsibilities
  • Implement physician-prescribed plans of care to ensure optimal patient outcomes
  • Conduct thorough patient assessments and coordinate care with healthcare teams
  • Administer prescribed medications, treatments, and therapies as ordered by physicians
  • Document care provided in a timely and compliant manner
  • Provide health education and training to patients and their families
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
Requirements
  • Active, unencumbered LPN license in the state of practice
  • Current CPR certification from a recognized organization
  • TB skin test (current within the last 12 months)
  • At least six months of prior hands-on nursing experience (preferred but not required)
  • Reliable transportation
