Job Summary

Aveanna Healthcare is seeking a compassionate and skilled LPN to provide high-quality care to our patients in the comfort of their own homes. As a Private Duty Nurse, you will work one-on-one with our patients, providing personalized care and support to help them achieve their goals and maintain their independence.

Responsibilities
  • Implement physician-prescribed plans of care to ensure our patients receive the best possible care.
  • Conduct patient assessments and coordinate care with our interdisciplinary team to ensure seamless transitions and optimal outcomes.
  • Administer prescribed medications, treatments, and therapies as ordered by our patients' physicians.
  • Document care provided in our electronic system of record, ensuring accurate and timely documentation.
  • Teach and train family members on health promotion, disease prevention, and management techniques to empower them to care for their loved ones.
  • Accept and write verbal orders from physicians as required, and obtain co-signature from our Case Manager on all verbal orders.
  • Follow up with physicians, clients, family members, staff, and Case Manager on client problems, concerns, and/or changes in client condition.
Qualifications
  • Active, unencumbered LPN license in the state where you will practice.
  • Current CPR certification from the American Red Cross, American Heart Association, or BLS.
  • TB skin test (current within the last 12 months).
  • Six months prior hands-on nursing experience preferred but not required.
  • Reliable transportation.
