This is a full-time position requiring 40 hours per week, with a flexible schedule from Monday to Saturday.

The Registered Nurse will typically work 3-4 shifts per week, including every other Saturday.

ROLE AND RESPONSIBILITIES:

The Outpatient Registered Nurse CAP 1 serves as an entry-level role within the Clinical Advancement Program (CAP). This position is responsible for delivering and coordinating high-quality clinical care, which includes assessment, planning, intervention, and evaluation for a designated group of patients. The RN may delegate appropriate tasks to direct patient care staff, including RNs, LVN/LPNs, and Patient Care Technicians. As part of the End Stage Kidney Disease (ESKD) healthcare team, the RN engages in decision-making, educational initiatives, leadership roles, and quality improvement efforts aimed at enhancing patient care outcomes and operational efficiency.

KEY DUTIES INCLUDE:

  • Executing essential functions under the supervision of the Nurse Supervisor, with support from Clinical Educators or in collaboration with fellow Registered Nurses.
  • Conducting ongoing, systematic collection and analysis of patient data before, during, and after hemodialysis treatment for assigned patients, documenting findings in the medical record, and making necessary adjustments to treatment plans while notifying the appropriate team members.
  • Assessing and documenting the educational needs of patients and their families to provide initial and ongoing education.
  • Overseeing and ensuring the safe and effective delivery of hemodialysis therapy in collaboration with both direct and ancillary patient care staff.
  • Administering medications as prescribed, ensuring proper documentation of medical justification and effectiveness.
  • Initiating or assisting with emergency response protocols as needed.
  • Serving as a resource for the healthcare team and participating in the training and orientation of new staff.
  • Ensuring compliance with laboratory collection, processing, and shipping procedures, and rescheduling any missed or insufficient laboratory collections.
  • Identifying expected patient outcomes and updating nursing assessments and care plans in collaboration with the Interdisciplinary Team.
  • Providing information to patients regarding transplant and treatment modality options.
  • Completing CAP requirements for career advancement.
  • Performing additional duties as assigned by the Supervisor.

PHYSICAL DEMANDS AND WORK ENVIRONMENT:

This role involves direct patient care, which may require heavy lifting, patient mobility assistance, and prolonged periods of standing. The employee may occasionally need to move equipment weighing up to 200 lbs. and lift solutions weighing up to 30 lbs. The work environment is typical of a healthcare facility, with controlled air temperature and moderate noise levels. There may be exposure to infectious materials.

SUPERVISORY RESPONSIBILITIES:

The RN may oversee Patient Care Technicians, LPNs, LVNs, or other RNs as a Team Leader or designated Nurse in Charge, contingent upon meeting specific educational and experiential requirements.

EDUCATION AND LICENSURE:

  • Graduation from an accredited School of Nursing.
  • Current state licensure as a Registered Nurse.
  • Current or successful completion of CPR BLS Certification.
  • Compliance with practice requirements in the state of employment.

EXPERIENCE AND SKILLS:

  • Entry-level position for RNs with less than one year of Nephrology Nursing experience in the past two years.
  • Preferred minimum of 9 months experience as a Registered Nurse.
  • Preferred experience in chronic or acute hemodialysis.
