Job Summary:
We are seeking a highly motivated and dedicated Public Health Specialist to join our team as an AmeriCorps Member. As a Community Health Worker II, you will play a critical role in promoting health equity and improving healthcare outcomes for underserved communities.
Key Responsibilities:
- Collaborate with community health workers, clinicians, and administrative professionals to provide technical assistance and mentorship to PHACHW I members.
- Provide screening, referral, and navigation services to connect individuals and their families to social and medical services.
- Develop and implement culturally competent care plans to address individual barriers and goals.
- Act as an intermediary between individuals and clinical and community service providers.
- Collect and maintain data on referrals and outcomes while adhering to established protocols and protecting patient confidentiality.
Requirements:
- High School Diploma or equivalent.
- Basic computer knowledge.
- Good judgment, initiative, and flexibility.
- Ability to develop positive relationships with others.
- Strong organizational and communication skills.
- Commitment to serving vulnerable populations.
- Bilingual (English/Spanish) preferred.
- 2 or more years of experience working in the community or healthcare facility.
- Promotor(a) or Community Health Worker Certification or willingness to obtain within six months of start.
