About the Role

At Elevance Health, we are seeking a skilled Palliative Nurse Practitioner (NP)/Physician Assistant (PA) to join our team. As a key member of our inter-disciplinary team, you will work closely with company physicians, patients' other physicians and providers, and their family members to develop complex plans of care in accordance with the patient's health status and overall goals and values.

Job Summary
  • Deliver clinical and non-clinical support to patients in palliative care within a designated territory
  • Develop and implement clinical plans of care for adult patients facing chronic and complex conditions
  • Gather history, perform physical exams, and conduct diagnostics as needed
Key Responsibilities
  1. Collaborate with healthcare professionals to develop personalized care plans
  2. Provide education to patients and families about medication usage, illness progression, and medical adherence
  3. Maintain accurate patient records and documentation
  4. Participate in continuing education and professional development
Requirements
  • Current, unrestricted NP or PA license in California
  • DEA registration or eligibility required
  • CPR certification
Preferred Qualifications
  • 2+ years of experience in home-based services, hospice, and palliative care
  • Bedside hospital, oncology, emergency room, or ICU experience preferred
  • Bilingual in English/Spanish
