Key Responsibilities

The Hospice Clinical Care Specialist will assist the agency's Executive Administrator, Executive Director, Director of Clinical Operations, and/or Patient Care Manager in ensuring the precision of patient electronic medical records and documentation of hospice care processes.
  • Facilitates communication with field staff, healthcare providers, referral sources, patients, and healthcare facilities. Takes messages and processes physician verbal orders as necessary. Ensures timely follow-up on all communications.
  • Regularly checks the status of incoming faxes and coordinates with the relevant personnel as needed.
  • Maintains professional competency through assigned online training and mandatory in-service sessions.
  • Assists with visit verification, scheduling, and planning of patient visits in accordance with established frequency orders.
  • Stays informed about the electronic medical record documentation system, patient care applications, and hospice agency operational tools such as Outlook, Teams, learning management systems, and policy manuals.
Qualifications

License Requirements:
  • Active LPN/LVN license in the state of practice.
  • Current CPR certification is mandatory.
  • Possession of a valid driver's license, vehicle insurance, and access to reliable transportation or public transit.
