Geriatric Care Coordinator

We are seeking a skilled Geriatric Care Coordinator to join our team at CAI. As a key member of our care coordination team, you will play a vital role in ensuring that our members receive high-quality, comprehensive care.
Job SummaryThe Geriatric Care Coordinator will be responsible for providing community-based care coordination for members in Delaware. This will involve assessing, planning, and coordinating care to meet the physical, behavioral, and social needs of clients. You will work closely with healthcare providers, caregivers, and other stakeholders to ensure seamless transitions of care and optimal outcomes for our members.
Responsibilities- Conduct face-to-face and telephonic evaluations to assess member needs and develop personalized care plans
- Coordinate care across the continuum of services, including hospitalization, rehabilitation, and long-term care
- Facilitate communication between healthcare providers, caregivers, and other stakeholders to ensure effective care coordination
- Educate members and caregivers about available resources and services to support their care
- Develop and implement back-up plans for gaps in provider coverage
To be successful in this role, you will need:
- A current Registered Nurse (RN) license in the state of Delaware
- At least 2 years of experience in case management and discharge planning, with a focus on geriatric care
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficiency in electronic documentation systems
Certified Case Manager (CCM) certification or other relevant certifications
