Nurse Specialist

Conexio Care is seeking a motivated and compassionate Registered Nurse to support individuals in their healthcare journey.
About the RoleThe Registered Nurse (RN) is a critical member of our clinical staff, providing medical expertise to multidisciplinary teams. As an RN at Conexio Care, you will work collaboratively with treatment teams and service providers to deliver effective health and wellness services to individuals with behavioral health challenges, developmental disabilities, and intellectual disabilities.
Duties and Responsibilities- Facilitate healthcare service needs of individuals served, ensuring these needs are addressed effectively.
- Complete all required medical assessments pursuant to best practices, contractual obligations, and licensure requirements.
- Maintain adherence to best practices, licensure requirements, and policies, procedures, and protocols of contractors and payers of services.
- Deliver effective wellness and medical services according to policies and procedures, state and federal regulations, and professional, ethical, and legal principles.
- Maintain an effective therapeutic alliance with individuals served.
- Communicate and coordinate with individuals receiving services, healthcare providers, family members or legal guardians regarding individuals' healthcare needs.
- Abridge by state and federal regulations, policies, and procedures related to confidentiality, patients' rights, and exposure and infection control.
- Provide after-hours on-call support to program staff.
- Encourage staff/clients while demonstrating self-initiative in problem-solving.
- Assist in the development of data collection and analysis of quality improvement projects.
- Ensure all client records are maintained according to agency standards, policies, and procedures.
- Engage in professional development, responsible for keeping current in the field, completing seminars and/or continuing education programs as necessary.
- Bachelor's Degree in Nursing or equivalent/associate degree.
- Delaware Registered Nursing License in good standing or Compact License.
- Certification in First Aid and CPR.
- Valid driver's license in good standing with at least 3 years minimum continuous driving experience with no serious traffic infractions.
- Experience with electronic health records preferred.
- Excellent communication skills and organizational skills.
- Ability to work with clients with substance abuse, co-occurring, and/or mental health disorders.
- Excellent writing skills and record-keeping skills.
- Detail-oriented.
- Knowledge of addiction, substance abuse treatment services, and counseling skills and theory.
