Job Summary

We are seeking a skilled and compassionate Case Manager to join our Adult Mental Health (MH) Services Case Management program. As a Case Manager, you will provide critical support to individuals and their families, helping them achieve their optimal level of functioning.

Key Responsibilities
  • Conduct thorough assessments and develop comprehensive service plans in collaboration with individuals and their representatives.
  • Coordinate necessary services, including referrals to public and private social services and community agencies.
  • Maintain accurate and complete individual records, producing clear and concise reports as needed.
  • Participate in ongoing training and professional development to enhance knowledge and skills.
  • Provide on-call services and respond to emergency situations as needed.
Requirements
  • Bachelor's degree in a relevant field, such as sociology, social work, or psychology, or equivalent experience.
  • Valid driver's license and reliable vehicle for travel to meetings and appointments.
  • Ability to work flexible hours and maintain a high level of professionalism in a fast-paced environment.
