Job Title: LVN Case Manager

Regal Medical Group is seeking a skilled LVN Case Manager to join our team. As a pivotal member of our interdisciplinary team, you will be responsible for identifying the physical, psychological, social, and spiritual needs of patients and their families.

You will initiate appropriate interventions and support for patients and families upon admission and provide a comprehensive and responsive plan of care. Your duties will include coordinating patient care and services, responding to necessary information, monitoring re-certification/re-authorization processes, and participating in interdisciplinary team meetings.

You will also ensure and document the appropriate use of medications, durable medical equipment, and supplies. Your role will involve educating patients, families, and caregivers, providing skilled nursing as determined by the interdisciplinary plan of care, and utilizing the skills and resources of contract personnel.

As a member of our team, you will be expected to integrate quality improvement results into the established plan of care, participate in routine QI assessments, proactive patient calls, and routine chart reviews. You will also participate in the orientation of new team members and attend regularly scheduled in-services and educational conferences.

Requirements:

  • Current/valid Vocational Nurse licensed in the State of California
  • Minimum of one (1) year experience as a professional nurse within the last three (3) years and holds a current Calif. LVN license
  • Previous experience in hospice or home health preferred
  • Knowledge of the principles and practices of primary medical and nursing care
  • BLS required
  • Demonstrates the ability to use good listening and communication skills
  • Able to multi-task effectively
  • Capacity to relate and work as a member of an IDT
  • Current California driver's license and proof of insurance
