Skilled Home Healthcare Professional Wanted

We are seeking an experienced and dedicated Licensed Practical Nurse (LPN) to join our team at Interim Healthcare - Houston. This role requires a strong passion for delivering high-quality patient care in a home health setting.
About the RoleThe ideal candidate will have a proven track record of providing excellent patient assessment skills, working effectively with minimal supervision, and possessing good time management and communication skills.
Key Responsibilities- Develop and implement personalized care plans tailored to each patient's unique needs
- Administer medication, perform wound care, treatments, and procedures as designated by the patient's healthcare plan
- Maintain accurate records of client observations, interventions, and evaluations to ensure seamless continuity of care
- Collaborate with our RN team to ensure comprehensive patient care and timely communication of any changes or concerns
- Hold an active LPN license in the State of Texas
- Possess current CPR Certification and updated PPD Test
- Have a minimum of one year of experience in a healthcare setting, preferably in home care, home health, hospital, or long-term care environments
- Pass a drug test and criminal background check prior to employment
