Managed Care Coordinator - Foster Care
Job Overview:
The Managed Care Coordinator for Foster Care is tasked with the comprehensive oversight of each member's personalized service plan, adhering to the relevant state regulations and contractual obligations.
Key Responsibilities:
Engage with children in the Foster Care System to conduct thorough assessments of their history and program requirements, utilizing structured tools with predetermined questions to evaluate, coordinate, and manage their needs effectively.
Identify members who may have significant clinical health care requirements, including potential high-risk complications, and collaborate with the clinical healthcare management team to ensure appropriate care coordination.
Address the non-clinical needs of members facing chronic conditions, co-morbidities, or disabilities, ensuring the efficient and cost-effective use of services.
Set both short-term and long-term objectives in partnership with members, their families, caregivers, and healthcare providers.
Recognize members who could benefit from additional services and travel to various locations as necessary to fulfill job duties.
Minimum Qualifications:
A Bachelor’s degree and at least one year of experience working directly with the specified program population or related community organizations; or a combination of education and experience that provides an equivalent background.
Preferred Qualifications:
A Bachelor’s degree in a healthcare-related field is preferred. Specific educational requirements and experience may vary based on state regulations and contractual stipulations.
For Managed Care Programs, Community Health Worker certification must be obtained within the first year of employment.
Experience with Foster Care is preferred, along with a background in Behavioral or Mental Health.
