Job Title: Case Manager

We are seeking a highly skilled Case Manager to join our team at ACI Federal Defunct. As a Case Manager, you will be responsible for providing service coordination and case management services to children with developmental delays and their families.

Key Responsibilities:

  • Assess, plan, link, monitor, and advocate for children and their families in response to their changing needs.
  • Complete clinical documentation in accordance with agency and regulatory guidelines.
  • Report to assigned supervisor and work collaboratively with other providers to ensure continuity of care.

Requirements:

  • Bachelor's Degree in human services or related field.
  • Two years' experience in the early intervention field working with infants/children with developmental delays.
  • Certification in Basic First Aid and CPR.

Working Conditions:

  • Must be able to lift at least 40 pounds.
  • May be required to transport clients in a minivan or car.
