We are seeking a highly skilled Case Manager to join our team at ACI Federal Defunct. As a Case Manager, you will be responsible for providing service coordination and case management services to children with developmental delays and their families.
Key Responsibilities:
- Assess, plan, link, monitor, and advocate for children and their families in response to their changing needs.
- Complete clinical documentation in accordance with agency and regulatory guidelines.
- Report to assigned supervisor and work collaboratively with other providers to ensure continuity of care.
Requirements:
- Bachelor's Degree in human services or related field.
- Two years' experience in the early intervention field working with infants/children with developmental delays.
- Certification in Basic First Aid and CPR.
Working Conditions:
- Must be able to lift at least 40 pounds.
- May be required to transport clients in a minivan or car.
