Position Responsibilities:
During a visit that may last up to one hour, our providers conduct thorough evaluations of a member's health status, medical history, medication compliance, social circumstances, and other potential risks. This process offers invaluable insights into a member's overall health that are often challenging to obtain during standard office visits. The Matrix Comprehensive Health Assessment is designed to enhance care quality and facilitate the closure of multiple care gaps in a single interaction.

Key Responsibilities:
- Conduct comprehensive Adult/Geriatric Assessments, including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and accurate documentation in accordance with Matrix protocols.
- Collaborate effectively with physicians, case managers, social workers, family members, and other relevant healthcare personnel.
- Engage with Primary Care Physicians (PCPs) for patient education and follow-up care.
- Build and maintain relationships within a nationwide network of advanced practice providers.
- Provide services in various settings, including home visits, skilled nursing facilities, and telehealth consultations.
Qualifications:
Essential Requirements:
- Master's Degree or equivalent experience with successful completion of NP licensure.
- Master's Degree or equivalent experience with successful completion of PA licensure.
- Current RN and NP/PA licensure in the state of practice, including prescriptive authority or the ability to obtain it.
- Board certification by NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty.
- Current BLS, ACLS, or CPR certification.
- A minimum of one year of experience as a Nurse Practitioner or Physician Assistant.
- Proficient computer skills and familiarity with EMR software.
- Excellent verbal and written communication skills with patients, clients, and colleagues.
- A positive, outgoing personality, and a collaborative team player who is adaptable to frequent changes.
Travel Requirements:
- Ability to travel, possess a valid state driver’s license, and maintain adequate automobile insurance coverage.
- Travel may be necessary.
