We are seeking a dedicated and experienced Physical Therapist to join our team at Royal Care CHHA in Nassau, NY. As a Physical Therapist with our company, you will have the opportunity to make a meaningful difference in the lives of our patients by providing exceptional care and support.

Our ideal candidate is a licensed Physical Therapist with a proven track record of success in home care settings. You should have excellent communication and interpersonal skills, with the ability to work independently with minimal supervision. A valid license to practice physical therapy and a graduate degree or greater in physical therapy are also required.

In this role, you will be responsible for assessing and understanding care plans, creating physical therapy treatment plans alongside physicians or by prescription, documenting visits and updates on patient care and progress, educating caregivers and their families on how to care for patients, making judgments on the types of care and services needed for each patient, and supporting our company mission by providing the highest quality of home care.

Requirements:

  • Proven work experience as a Physical Therapist
  • Graduate degree or greater in Physical Therapy
  • Valid license to practice Physical Therapy
  • Bilingual skills are a plus
  • At least 1-year experience as a Physical Therapist in home care - Medicare Part A experience
  • Reliable transportation
  • Great communication, organizational, and time management skills
  • Problem-solving skills
  • Interpersonal skills
  • Excellent computer skills
  • Able to work independently with minimal supervision
