We are seeking a skilled and compassionate Registered Nurse Case Manager to join our team at Interim HealthCare of New Hampshire. This is an excellent opportunity for a dedicated nurse to provide high-quality patient care in the comfort of patients' homes.
About the RoleThe RN Case Manager will be responsible for managing a caseload of patients, providing direct care and comprehensive nursing services to individuals in the community. With a focus on flexibility and autonomy, this role allows nurses to create their own schedule and work-life balance.

Key Responsibilities- Provide exceptional patient care and support in patients' homes
- Develop and implement personalized care plans tailored to each patient's needs
- Collaborate with healthcare professionals to ensure seamless care coordination
- Conduct thorough assessments and evaluations to identify patient needs and goals
- Maintain accurate and up-to-date records of patient interactions and progress
To be successful in this role, you will need:
- A valid Nursing License in the State of New Hampshire
- Nursing experience required, preferably in home health or a related field
- Current CPR Certification (we offer monthly renewal classes)
- Reliable transportation
