About the Role:

As a member of our Transition Team, you will provide care for all authorized hours in a home during a provisional period of 1-90 days.

During this time, our office staff will be recruiting full-time, replacement home healthcare nurses.

You will have the opportunity to train and shadow new caregivers in the home before they work independently with the patient.

Once the replacement nurses are trained and have a schedule, you will transition to another new admission.

Responsibilities:

  • Utilize the nursing process to plan, implement, and evaluate patient/client care and/or services provided to clients receiving contracted services.
  • Initiate the plan of care when directed and request necessary revisions as patient status and needs change.
  • Incorporate therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals.
  • Intervene in response to changes in patient/client condition and report such changes to the appropriate physician or supervisor.
  • Administer medications and treatments as prescribed, and monitor and document the effectiveness of interventions.
  • Review the plan of care and program protocols, identifying areas for healthcare teaching/education, care coordination, goal setting, and discharge planning needs.

Requirements:

  • Current Licensed Practical Nurse (LPN) License for the state in which you practice.
  • Current PPD or Chest X-Ray.
  • Current BLS card.
  • One year of prior LPN experience preferred.
