Job Summary

The Licensed Practical Nurse (LPN) plays a vital role in providing high-quality care to residents in our long-term care facility. As a member of our interdisciplinary care team, you will work closely with healthcare professionals to deliver compassionate and individualized care to our residents.

Key Responsibilities:

  • Provide direct care to residents, including administering medications, performing treatments, and documenting resident interactions.
  • Contribute to the assessment of residents and develop care plans that meet their unique needs.
  • Collaborate with the care team to ensure seamless transitions and effective communication.
  • Provide education and support to residents and their families on prevention and treatment plans.
  • Participate in quality improvement initiatives to enhance resident care and satisfaction.

Requirements:

  • Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
  • Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
  • Obtains and subsequently maintains required department specific competencies and certifications.
