Job Summary

The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long term care, under the supervision of a registered nurse, advanced practice provider, or physician. This role requires a strong understanding of nursing principles and the ability to provide high-quality care to residents with diverse needs.

Key Responsibilities
  • Provide direct care to residents, including administering medications, treatments, and performing various nursing procedures.
  • Conduct thorough assessments of residents to identify their physical and psychosocial needs.
  • Develop and implement individualized care plans to meet the unique needs of each resident.
  • Collaborate with the interdisciplinary care team to ensure seamless care delivery and effective communication.
  • Document resident interactions, outcomes, and care provided in a timely and accurate manner.
Requirements
  • Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
  • Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
  • Obtains and subsequently maintains required department specific competencies and certifications.
