About the Role:

Matrix Medical Network is seeking a skilled Clinical Health Specialist to join our team. As a Home Risk Assessment Professional, you will play a vital role in improving the quality of care for our patients. Your expertise as a Comprehensive Health Care Provider will enable you to conduct thorough assessments, identify potential health risks, and develop effective care plans.

Key Responsibilities:

• Conduct comprehensive health assessments in patients' homes, skilled facilities, or through telehealth visits.
• Review medical history, medication adherence, social environment, and other risk factors to identify potential health concerns.
• Develop and implement personalized care plans to address identified health risks and improve patient outcomes.
• Collaborate with healthcare providers, case managers, and other stakeholders to ensure seamless care coordination.
• Provide education and support to patients and their families to promote healthy behaviors and disease management.

Requirements:

• Master's degree in a relevant field (e.g., nursing, healthcare administration) or equivalent experience.
• Current licensure as a Nurse Practitioner or Physician Assistant in the state of practice.
• Board certification in a relevant specialty (e.g., family medicine, gerontology).
• Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
• Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments.
• Ability to travel to patients' homes, skilled facilities, or other locations as needed.
