We are looking for a dedicated Licensed Practical Nurse (LPN) to take on the role of Patient Services Coordinator within our organization. This position is essential for overseeing visit scheduling, coordinating patient care, and managing the daily operations of our office. The successful candidate will engage with field personnel, patients, healthcare providers, and other relevant parties to ensure effective care coordination and seamless continuity of services.

  • Oversee patient schedules, including adjusting appointments in the event of staff unavailability.
  • Initiate and manage infection control documentation and maintain a log of client hospitalizations.
  • Support weekly case discussions and facilitate internal patient transfers among branch locations.
  • Generate scheduling reports and fulfill essential scheduling responsibilities.
  • Conduct patient visits and participate in on-call duties if clinically qualified.

Essential Qualifications and Skills:

  • Active LPN or LVN license in the relevant state.
  • A minimum of one year of experience in home health care.
  • Excellent organizational and communication abilities.
  • Preferred: Coding certification and experience with packet reviews.

Work Schedule and Compensation:

This is a full-time role requiring 40 hours per week. The salary range is between $37,200 and $51,200 annually, depending on experience and location.
