We are seeking a skilled and compassionate Registered Nurse to join our team at BrightSpring Health Services in Rutherford, TN. As a Home Health RN, you will have the opportunity to provide high-quality patient care in a dynamic and supportive environment.
Key Responsibilities- Provide safe and timely patient care using evidence-based nursing practices and best practices as defined by our quality assurance team.
- Participate in weekly team meetings to coordinate patient care and manage COVID-19 safety measures.
- Develop and implement individualized nursing care plans to facilitate patient rehabilitation and achieve quality outcomes.
- Provide quality, compassionate nursing care while collaborating with other members of the interdisciplinary treatment team.
- Monitor and adjust patient care plans as needed to ensure optimal patient outcomes.
- Administer medications as ordered by the patient's Primary Care Physician and provide medication instruction and monitoring.
- Conduct patient assessments and develop care plans with other team members.
- Supervise Licensed Practical Nurses and Home Health Aide staff as needed.
- Document patient care and communicate changes in patient needs to the physician and nursing supervisor.
- Registered Nursing Degree (Associate or Bachelor) from an accredited college of nursing with current unrestricted registration and license in the applicable state.
- One year of nursing practice in a patient care setting, with experience in home health, geriatrics, or other related settings preferred.
- Valid driver's license, acceptable driving record, and proof of car insurance.
- Current CPR certification.
