Job Title: Primary Care Nurse Practitioner

At Olesky, we are seeking a highly skilled Primary Care Nurse Practitioner to join our team at the Family Health Center in Newark, NJ.

Job Summary:

The Primary Care Nurse Practitioner will play a critical role in managing the medical needs of patients, providing acute, preventive, and chronic disease care, as well as care management and individualized treatment plans.

Responsibilities:
  • Perform physical examinations and preventive health measures within prescribed guidelines and instructions of Physician Orders.
  • Interprets, and evaluates diagnostic tests to identify and assess patients' clinical problems and health care needs.
  • Record physical findings, and formulates plan and prognosis, based on patient's condition.
  • Provide written home-going instructions.
  • Collaborate with Physician and other health professionals to prepare comprehensive patient care plan as necessary.
  • Prescribe or recommend medications or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures.
  • Refer patients to Physician for consultation or to specialized health resources for treatment.
  • Maintains medical records (Using Electronic Medical Record software).
  • Educate and coach nursing staff on best nursing practices.
  • Communicate with and counsel family members on treatment options material changes in status.
Requirements:

The ideal candidate will have a strong background in primary care and a passion for delivering high-quality patient care. A Master's degree in Nursing and current APRN licensure are required.
