About the Role

We are seeking a highly skilled Integrated Behavioral Health Specialist to join our team at Southwest Montana Community Health Center. As a key member of our healthcare team, you will provide behavioral health services to a diverse patient population, working under the direction of our Director of Behavioral Health.

Key Responsibilities
  • Provide behavioral health services to patients, including assessment, diagnosis, and treatment planning.
  • Collaborate with primary care providers to coordinate care and ensure comprehensive treatment.
  • Develop and implement individualized treatment plans, taking into account patient needs and goals.
  • Conduct regular assessments and evaluations to monitor patient progress and adjust treatment plans as needed.
  • Provide education and support to patients and their families, promoting healthy behaviors and coping strategies.
Requirements
  • Master's degree in a relevant field, such as social work, counseling, or psychology.
  • Current licensure as an LCSW, LCPC, or candidate licensure in good standing.
  • Previous experience working in a team setting or integrated behavioral health model is preferred.
