Community Health Advocate

Job Summary:
As a Community Health Advocate with LifeBridge Health, you will play a vital role in extending psychosocial support services to individuals and families in need. Your primary responsibility will be to provide direct services through education, advocacy, and services coordination to empower clients to overcome psychosocial barriers to health and well-being.
Key Responsibilities:
- Conduct thorough assessments of individual and family needs to develop personalized service plans.
- Provide education and psychosocial support on various topics, including health, medical compliance, budgeting, safety, parenting, and community resources.
- Collaborate with clients to identify and address psychosocial barriers to health and well-being.
- Develop and maintain relationships with community partners to ensure seamless service delivery.
- Participate in ongoing training and professional development to stay current on best practices in community health advocacy.
Requirements:
- Associate's degree or equivalent formal working knowledge in a specialized field.
- 1-3 years of experience in a related field, preferably in healthcare or social services.
- Driver's license and personal transportation preferred for some service areas.
