Case Manager I

The FRT Case Manager serves as a vital team member with social service professionals and paraprofessionals to deliver quality care and uphold the mission of St. Joseph Center. This role focuses on providing services to homeless families on the Westside of Los Angeles.
Key Responsibilities- Act as the primary point of contact for families within the homeless family solutions system.
- Screen homeless families referred through Los Angeles County 211 using a standard prescreening triage tool.
- Negotiate with family, friends, and landlords to prevent families from entering the homeless system.
- Connect families to supportive services, including health, mental health, public entitlements, education, and vocational training.
- Advocate or liaison for families with community organizations.
- Complete client intakes and work closely with the Problem Solving team.
- Highly motivated and self-starting individual with excellent communication and interpersonal skills.
- Working knowledge of the housing market and housing programs is desirable.
- Familiarity with mental illness and substance abuse issues is strongly preferred.
- Ability to work flexible hours, including early mornings and evenings.
- Bachelor's degree in social work, human services, or a related field.
- One year of experience providing case management services to at-risk populations.
The work environment will be primarily field-based, with occasional office hours. The employee must be able to stand, sit, walk, stoop, talk, hear, reach above and below shoulders, and use hand and finger dexterity. The employee may be required to lift and carry up to 20 pounds.
