About the Role:

Bellefaire JCB, a leading child service agency, is seeking a skilled Care Coordination Specialist to join our team. As a key member of our Care Management Entity (CME), you will play a vital role in connecting clients with essential services through OhioRISE.

Responsibilities:

  • Initiate clinical assessments to determine the most suitable level of treatment for clients
  • Provide face-to-face admissions with clients and guardians, ensuring all required paperwork is completed
  • Make recommendations for community or national agencies when Bellefaire JCB cannot provide the necessary services
  • Manage a diverse caseload of referrals, representing the Agency's service array
  • Assist with departmental coverage of incoming referrals, outreach, and CANS assessments

Qualifications:

  • Bachelor's Degree required; Master's Degree in Social Work or related field preferred
  • Valid Ohio LSW, LISW, or PCC license preferred
  • At least 3 years of experience in the mental health field; experience working with youth and families preferred
