Key Responsibilities- Conduct thorough assessments to identify clients' strengths, needs, preferences, and barriers to independent living
- Gather relevant information from clients, families, caregivers, and other service providers to develop a comprehensive understanding of the client's situation
- Liaise with various community-based organizations, healthcare providers, government agencies, and other stakeholders to coordinate services and resources for clients
- Facilitate referrals to external agencies or specialists for medical, behavioral health, housing, employment, education, or other supportive services as needed
- Other job duties as assigned
- Associate degree or 2-5 years in a related field required. Bachelor's degree preferred
- Valid NYS driver's license with driving record acceptable to BHSN insurance carrier
- Reliable transportation for required travel
- Ability to work a flexible schedule to accommodate the needs of families
- $58,000 - $68,000 per year, depending on experience
