We are a dynamic team dedicated to providing outstanding in-home patient care in the North Houston area.

Role Overview

This full-time W2 role involves visiting patients in their homes for medical evaluation and management, with a competitive salary of $101,000–$108,000 per year. The position requires residing in the Woodlands, Spring, Tomball, or nearby areas, with applicants needing to include their home address in the application.

Responsibilities

  • Diagnose and manage acute, chronic, and episodic conditions.

  • Prescribe medications and treatments.

  • Perform thorough physical examinations.

  • Develop and implement patient management plans.

  • Facilitate referrals and coordinate care with home health agencies.

  • Supervise and adjust home health services as needed.

Support and Training

  • Providers will be assigned a dedicated Medical Assistant to handle administrative tasks such as route mapping, confirming patient visits, phone calls, and order processing.

Requirements

  • Certified Nurse Practitioner or Physician Assistant license required.

  • At least 1 year of experience as an advanced practice provider (preferred).

  • Spanish-speaking candidates are highly desirable.
